Focal Liver Lesions in the Non-Cirrhotic Liver

چکیده
1. Hepatic Cyst Hepatic cysts are common benign lesions that occur in 2%-10% of the population. They may be solitary or multiple and range in size from a few millimeters to several centimeters. Hepatic cysts are usually discovered incidentally and have no malignant potential. On MR imaging, they are well-defined, round or oval lesions with thin, almost imperceptible walls. Simple cysts are very hypointense on T1-weighted sequences and very hyperintense on T2-weighted sequences, corresponding to the signal characteristics of fluid. Additionally, simple cysts demonstrate no internal architecture and no gadolinium contrast enhancement. Biliary hamartoma may be indistinguishable from simple hepatic cysts but are also benign entities. Indeterminate subcentimeter hypodensities seen on CT in patients with known primary carcinomas can usually be definitively diagnosed on MRI.

Diagnosis of well-differentiated hepatocellular lesions:
The most common problems encountered in the differential diagnosis of hepatocellular lesions are: (a) adenoma vs well-differentiated hepatocellular carcinoma (HCC) in non-cirrhotic liver, (b) focal nodular hyperplasia (FNH) vs adenoma in non-cirrhotic liver, and (c) HCC vs high grade dysplastic nodules and other non-neoplastic nodules in cirrhotic liver. This discussion provides an update of im...
متن کاملFocal lesions in cirrhotic liver: what else beyond hepatocellular carcinoma?
Detection and characterization of focal lesions in the cirrhotic liver may pose a diagnostic dilemma. Several benign and malignant lesions may be found in a cirrhotic liver along with hepatocellular carcinoma (HCC), and may exhibit typical or atypical imaging features. In this pictorial essay, we illustrate computed tomography and magnetic resonance imaging findings of lesions such as simple bi...
متن کاملWhat proportions of focal liver lesions detected by unenhanced ultrasound are inconclusive?
In August 2012, the National Institute for Health and Care Excellence produced positive diagnostics guidance on the ultrasound contrast agent SonoVue®, but recommended further research involving an estimation of the proportion of unenhanced ultrasound scans reporting, but not characterising, focal liver lesions, particularly in cirrhotic livers. Patient records from the Radiology Information Sy...
متن کاملMRI of Focal Liver Lesions
Magnetic resonance imaging, MRI has more advantages than ultrasound, computed tomography, CT, positron emission tomography, PET, or any other imaging modality in diagnosing focal hepatic masses. With a combination of basic T1 and T2 weighted sequences, diffusion weighted imaging, DWI, and hepatobiliary gadolinium contrast agents, that is gadobenate dimeglumine (Gd-BOPTA) and gadoxetic acid (Gd-...

• HCC is the most important differential diagnosis for focal liver lesions in the cirrhotic liver • Detection of HCC is crucial to ensure early therapy and proper patient management • Challenging differential diagnoses for HCC are mainly regenerative nodules and pre-neoplastic nodules • MRI provides a multiparametric approach to detect and characterize HCC – including MR signal in T2w images an...
متن کاملImaging findings of mimickers of hepatocellular carcinoma
Radiological imaging plays a crucial role in the diagnosis of hepatocellular carcinoma (HCC) as the noninvasive diagnosis of HCC in high-risk patients by typical imaging findings alone is widely adopted in major practice guidelines for HCC. While imaging techniques have markedly improved in detecting small liver lesions, they often detect incidental benign liver lesions and non-hepatocellular m...
